If you are a mother-to-be and enrolled in one of the two State Health Plans, you are encouraged to enroll and participate in the free maternity management program: Coming Attractions.  More information and instructions for enrollment are located on page 51 of the 2023 Insurance Benefits Guide.
 
If you are planning to add your baby to your insurance, please log into your MyBenefits account and complete the change request after he/she is born, but WITHIN 31 DAYS OF THE DATE OF BIRTH.  A long-form birth certificate will be required and can be obtained from the local health department after he/she is at least 5 days old.  It is recommended that you enter this request as soon as practical, even if you have not received the birth certificate, so that you don't miss the 31 day deadline.  If your insurance premiums change, you will be contacted by someone in the Benefits Office.  IMPORTANT: If your maternity leave results in reduced or missed paychecks, an electronic invoice will be sent to your Anderson 5 email address each pay period.
 
 
 
*Short Term Disability While on Maternity Leave
 
If you are enrolled in short-term disability insurance with Colonial Life, it will pay you for six weeks for a normal delivery and eight weeks for a cesarean section delivery.  Your premium and weekly benefit are based on the category in which you are enrolled.  Sick leave days are NOT an offset to this coverage, so this benefit will pay even if you have sick days to cover your absence.  Benefits are paid directly to you.
 
If you are not sure if you have this coverage, log into your Colonial Life Member Services account or contact Customer Service directly by calling 1-800-325-4368.
 
Once you apply for Family Medical Leave by submitting the *Notification of Need for Leave of Absence form (below), a claim form will be sent to you if you are enrolled in this coverage.  If you have any questions, contact the Benefits Office at (864) 260-5000, ext. 10103.

This form should be submitted, along with a doctor's statement, prior to your entering maternity leave.  It can be accessed through the District Intranet under "Administrative Documents" from a district computer.   Please see the FMLA FAQs page for additional information.
