Name/Address Change Instructions
Contact information should be kept current in the Benefits Office at all times.
To change your address, please access the links below:
MyBenefits (changes your address, phone number(s), and/or email address with PEBA's insurance system)
Member Access (changes your address, phone number(s), and/or email address with PEBA's retirement system)
SC Deferred Compensation (changes your address, phone number(s), and/or email address with Empower Retirement for employees enrolled in the 401(k) and 457(b) accounts)
Colonial Life (changes your address, phone number(s), and/or email address with Colonial for employees with whole life and/or cancer policies)
Or, you may change your contact information by sending a completed copy of the applicable forms below to the Benefits Office:

If you are a member of the SC Optional Retirement Plan, please contact your ORP Vendor to have your address changed on their records as well. You may also use the websites below:

If your name has changed, please contact the Benefits Office to set-up an appointment to complete this process. (Additional documentation will be required such as a copy of your updated social security card AND one of the following: marriage license, divorce decree, and/or other court document showing change of name.)
